About eBrightHealth ACO
eBrightHealth ACO is an accountable care organization which serves Medicare beneficiaries from Delaware, Pennsylvania, Maryland and New Jersey. Representing a broad statewide coalition, eBrightHealth ACO brings together primary and specialty care clinicians from regional health systems, federally qualified health centers and private primary care practices.
Through highly coordinated care, this collaboration helps clinicians address the overall health needs of Medicare beneficiaries, including behavioral and social health, along with careful management of chronic conditions which prevents the need for emergency care and hospitalization.
eBrightHealth ACO is supported by the physician-led, ChristianaCare Clinical Alliance and care coordination services are provided by ChristianaCare’s care coordination company, CareVio.
